School health services are procedures established
to:

appraise the health status of pupils

counsel pupils, parents, and others concerning appraisal
findings

encourage the correction of remediable defects

assist in the identification and education of handicapped
children

help prevent and control disease

provide a healthful school environment
-
provide emergency service for injury or sudden illness

Medication Policies
All
prescription medication must be kept in the clinic in the original
container with a complete and accurate prescription label. When a
prescription label is not legible or a dosage change is requested, a
written physician order will be required. All prescription medication
must be transported to and from the clinic by a parent/guardian.
Students are not allowed to carry prescription medication on campus.
All
medication brought to the clinic should be in the original container,
clearly labeled with the student's name, and accompanied by written
parental permission. Over the counter medication will be dispensed in
accordance with labeling directions. Please consult the label for your
child's correct dosage and recommended duration of treatment. Over the
counter or non-prescription medication needed for longer than a two-week
time period will require a written request from a physician.
Please
note that due to the dangers of REYE'S Syndrome, no aspirin or
aspirin-containing products will be administered by clinic staff unless
expressly ordered by your child's physician . In addition, the clinic
staff will not administer vitamins or herbal preparations.

Administering Medication
HISD
school personnel are not permitted to give medication of any kind,
including aspirin, similar preparations, or any other drugs. If
prescription medicine is required, it must be in its original container,
kept in locked storage in the office of the school nurse or of the
principal's designee, and administered by the nursing staff or a school
employee. The school may accept a sufficient quantity of medicine for
one month. Upon receipt, the medicine will be inventoried and
discrepancies reported to parents.
Over the
counter medications may only be kept at the school for two weeks unless
a doctor's order is provided to the health clinic.
Medication with limited duration will be administered for the time
prescribed on the label, and any leftover medicine not picked up by the
parent will be destroyed after two weeks.
IMMUNIZATION REQUIREMENTS: (Ages 7 years and older)
Two
doses of measles vaccine for students born on or after 9/21/91. For
students born on or after 9/1/78, a second dose of m easles vaccine is
required by 30 days after the child's 12th birthday. Serological
confirmation of measles immunity will satisfy either requirement.

One dose of
rubella vaccine on or after the 1st birthday or serologic
confirmation of rubella immunity.
-
One dose of
mumps vaccine on or after 1st birthday or serologic confirmation of
mumps immunity.
-
Three doses
of polio vaccine including one received on or after fourth birthday.
-
Three doses
of DTP/DTap/Td including one received on or after fourth birthday
and one dose within the past 10 years.
-
Children born
on or after September 2, 1992 must have three doses hepatitis B
vaccine. Children born between September 2, 1988 and September 1,
1992 (inclusive) must show proof of three doses of hepatitis B
vaccine within 30 days of their 12th birthday.

Children born
between September 2, 1988 and September 1, 1994 (inclusive) must
have one dose of varicella received on or after 1st birthday and by
at least 30 days after their 12th birthday. Serologic proof of
immunity or documentation of previous illness may substitute for
vaccination. Previous illness may be documented by a written
statement from a physician, school nurse or the child's/student's
parent or guardian containing wording such as: "This is to verify
that (name of student) had varicella disease (chicken pox) on or
about (date) and does need a varicella vaccine."

TB skin test:
Students entering public school for the first time and students who
have resided outside the United States within the past year must
have proof of a TB skin test(PPD) within the past year with a result
validated 48-72 hours after testing.

The above are
minimum immunization requirements as set by the Texas Department of
Health. Immunizations may be required up to 6 months before the
delinquent date.

Student
immunizations must be current in order for a student to attend
classes. If you receive a notice that your child is due for an
immunization please get the immunization as soon as possible and
send the verification to the school clinic.
SCREENINGS
Vision
screenings are routinely administered to all 7th grade students and any
students new to Clifton during the year. If a student fails the vision
screening, a referral letter is mailed home and you are asked to have
your student seen by an eye care specialist if they have not been seen
within the last year. The "eye examination report" should be taken with
you to the examination, filled out by the eye specialist, and returned
to the clinic as soon as possible. The clinic is required to keep
current vision examination results in your child's health record.
Hearing screenings are also
routinely administered to all 7th grade students and any students new to
Clifton during the year. If a student fails the hearing screening, a
referral letter is mailed home and you are asked to have your student
seen by an appropriately-licensed professional for further evaluation.
When your student is examined, please ask the professional to complete
the "hearing referral form" and return the form to the clinic as soon as
possible.
Scoliosis screenings are
routinely administered to all 8th grade students and any students new to
Clifton during the year. If a student fails the scoliosis screening, a
referral letter is mailed home and you are asked to have your student
evaluated by the physician of your choice. The completed form needs to
be returned to the clinic as soon as possible.
